How do I access my Report Card? 

Reports Cards can be found here: https://swimgen.net/search/CGACSJ 

• You will be prompted to enter your participant number. This number can be found in your Perfect Mind account.
• Log in to your Perfect Mind account. After logging in, locate the "My Info" section.
• Inside the "My Info" section, you should see a list of names. Select the name associated with the report card you want to access.
• Within the participant's details, click on "General Info."
• In the "General Info" section, you will find the "Contact ID." This is your participant number (do not include the CO-).
